About Our Continuous Care Program

We know that dealing with chronic eye conditions can be overwhelming. We’re here to help.

We provide you with a dedicated Care Coach who works with your doctor to make sure you get the best eye care possible.

This program is intended to support you as you navigate both your eye condition and your health conditions by making it easier for you to get the right care at the right time, and achieve better health outcomes. This program does not replace any of your current care, and our practice will remain your contact in the event of acute, emergent, or urgent conditions.

What Our Care Coaches Can Do For You

Call you in between office visits to check-in about your vision and overall health

Help you better understand your doctor’s instructions and answer your questions

Provide resources to help you better manage your symptoms

Help you find prescriptions, vitamins, or other items essential to your vision health at the lowest cost

Assist with finding a ride to your appointment, filling out paperwork, or refilling your prescription(s)

This program is typically no cost to you if you have Medicare insurance with supplemental coverage or managed Medicare.

What if I'm doing fine on my own?

Our goal is to help you achieve and maintain good vision health. That’s why the program focuses on preventive care and connecting you to valuable healthcare resources and community services.

Is this a long term commitment?

No, you may opt-out any time by notifying us via phone.
